Submit My PM&C Report

There are three ways to submit the Professional Maintenance and Certification (PM&C) report.

Microsoft Excel:

This version of the PMC report is an Excel spreadsheet with linked cells. To download and use a copy of this report, right-click here and select "download file" or "Save Target As". It is important that you do not try to run this file from its present location, as that will not work and might cause your report to become unstable. Please note that some of the cells are password protected. If you need assistance on filling out this report, please contact Chris Stelzig.

PDF:

The report is due on March 15th of the year following the third year covered in the report (e.g., a report due on March 15th, 2010 would cover 2007, 2008, and 2009). For new BCEs, if you were certified in the first half of a year, your "start date" for accumulating continuing education unites (CEUs) would be January 1st of that year. If you were certified in the second half of the year, your start date for CEUs would be January 1st of the following year. Your PM&C report should be submitted to the following address:

BCE-Administrative Recertification option:
For BCEs whose job duties have become more administrative and less applied, there is an option to convert to BCE-Administrative. The requirements to renew are the same, but the composition and types of CEUs required vary. Click here for more information.
